Assessment

Is there an exam for this course?

No. Root Cause Analysis training is a non-certification skills course with no exam. Learning is reinforced through hands-on tool exercises, case-study investigations and team discussion throughout the one-day programme.

Will I receive a certificate?

Yes. You receive a course completion certificate from Invensis Learning after finishing the programme. It confirms you have completed practical training in the full RCA toolkit.

How is learning assessed during the course?

There is no formal assessment. Instead, you apply each tool to realistic scenarios through guided exercises, including an Is/Is Not activity, a 5 Whys investigation, a Fishbone workshop and an 8D report, so you practise the skills as you learn them.

Is this a recognised certification?

No. This is a practical skills training programme with no awarding body or exam. The value is in the immediately usable problem-solving capability you build, which you can apply at work straight away.

Course Content

What topics does the RCA course cover?

The programme covers RCA principles and the three levels of cause, problem definition and data collection, the 5 Whys, the Fishbone diagram and advanced tools such as Fault Tree Analysis and FMEA, the full 8D methodology, and corrective action, verification and building an RCA culture.

What problem-solving tools will I learn?

You will learn five core tools and when to use each: 5 Whys for simple problems, the Fishbone diagram for complex multi-cause issues, Fault Tree Analysis for safety-critical events, FMEA for risk prioritisation, and 8D for team-based corrective action.

How long is the training?

The course runs for one day of about eight hours, covering all six RCA modules with concept explanations, worked examples and hands-on exercises. An extended format spread over two half-days is also available where that suits your schedule.

Is the course hands-on or theory-based?

It is highly hands-on. You apply every tool to realistic problem scenarios and case studies during the day, including completing an 8D report, so you leave able to run investigations rather than just describe them.
