What Root Cause Analysis Is and Why It Pays off in ZambiaUntitledA Practical One-Day Programme

Root Cause Analysis (RCA) is a structured, systematic approach to problem solving that finds the fundamental cause of a defect, failure or incident, not just the symptom, so the problem is eliminated for good. Root cause analysis training in Zambia equips you with a complete toolkit, the 5 Whys, Fishbone (Ishikawa) diagram, Fault Tree Analysis, Failure Mode and Effects Analysis and the 8D methodology, and shows you when to apply each one. This one-day, instructor-led programme is built for quality engineers, production supervisors, maintenance and reliability teams, health and safety officers and continuous improvement practitioners across Zambia's copper mines, manufacturers and agro-processors. You learn to define a problem precisely, gather evidence, drill to the true root cause and put corrective actions in place that prevent recurrence. RCA sits at the heart of ISO 9001:2015 corrective action (Clause 10.2), the Six Sigma DMAIC Analyse phase and modern incident investigation, so the skills apply on the plant floor from day one. Start building capability your employer can see in fewer repeat failures, with Invensis Learning.

Why RCA Skills Are in Demand in Zambia

    Repeat Failures Erode Output

Copper mines and processors lose costly production to the same breakdowns and defects. Without structured investigation, teams treat symptoms and the failure returns. RCA targets the underlying cause.

    RCA stops problems recurring

    Copper Push Demands Reliability

Zambia's drive toward one million tonnes of copper leaves little room for unplanned downtime. Equipment reliability and defect reduction now depend on disciplined root cause investigation.

RCA builds failure and reliability skills

    ISO 9001 Corrective Action Pressure

Exporters and suppliers must show real corrective action under ISO 9001:2015 Clause 10.2. Auditors expect evidence of root cause investigation, not quick fixes, which many teams struggle to provide.

    RCA supports Clause 10.2 compliance

    Cost and Margin Discipline

With copper prices and input costs under pressure, rework, scrap and warranty claims hit margins directly. Eliminating root causes protects profitability across operations.

RCA cuts the cost of poor quality

    Mining and Plant Safety

Safety-critical incidents and near misses in mining and heavy industry demand formal, structured investigation. Tools such as Fault Tree Analysis reveal the systemic causes behind unsafe events.

    RCA strengthens incident investigation

    Structured Problem-Solving Gap

Many Zambian operations rely on experience and firefighting rather than a common method. A shared RCA toolkit lets quality, production and maintenance teams solve problems consistently.

RCA gives teams a shared method

Recurring failures quietly erode output and margins across Zambian industry. Root cause analysis skills turn that around. Build the capability to find and fix problems at the source. Start today.

    General

    What is root cause analysis?

Root cause analysis is a structured, systematic approach to finding the fundamental cause of a problem, defect or failure, rather than just its symptom. It uses proven tools such as the 5 Whys, Fishbone diagram, Fault Tree Analysis, FMEA and 8D so that corrective actions prevent the problem recurring.

Is RCA training worth it for professionals in Zambia?

Yes. Recurring failures cost Zambian mines, manufacturers and processors output and margin, so employers value people who can investigate problems properly and stop them returning. The skills apply across quality, production, maintenance, safety and improvement roles.

Who should attend this RCA training?

The programme suits quality engineers and managers, production supervisors, maintenance and reliability teams, health and safety officers, and continuous improvement practitioners. Anyone responsible for investigating problems or corrective action will benefit, and there are no prerequisites.

Yes. You receive a course completion record from Invensis Learning after finishing the one-day programme. The training builds a practical RCA toolkit you can apply immediately, and does not award a formal external credential.

How is learning assessed during the course?

There is no formal assessment. Instead, you apply the tools through practical exercises, an Is/Is Not problem-definition activity, a 5 Whys investigation, a Fishbone workshop and a full 8D report on a realistic problem.

    Is RCA a recognised discipline?

Yes. Root cause analysis is a core discipline across quality management, Six Sigma, Lean, healthcare patient safety and IT problem management, and it is central to ISO 9001 corrective action. The skills are recognised in every industry that must prevent problem recurrence.

Can beginners complete the training successfully?

Yes. The programme teaches each RCA method from first principles with guided practice, so learners with no prior background can apply the tools by the end of the day.

    Course Content

What tools does the RCA training cover?

The course covers the 5 Whys, the Fishbone (Ishikawa) diagram, Fault Tree Analysis, Failure Mode and Effects Analysis in the RCA context, and the 8D problem-solving method, plus Is/Is Not analysis, Pareto and problem definition. You also learn when to use each one.

How long is the RCA training programme?

The programme runs for one day (eight hours), covering RCA principles, problem definition and data collection, the 5 Whys, Fishbone and advanced tools, the 8D method, and corrective action and verification, with hands-on exercises throughout.

What will I be able to do after the training?

You will be able to define a problem precisely, run 5 Whys and Fishbone investigations, apply Fault Tree Analysis to safety-critical events, complete an 8D report, and develop and verify corrective actions that prevent recurrence.

    Are there hands-on exercises?

Yes. Practice runs throughout the day, including an Is/Is Not problem-definition exercise, a 5 Whys investigation, a Fishbone workshop, and completing an 8D report for a realistic scenario, so you apply every method during the session.

Does the course cover ISO 9001 and CAPA?

Yes. The training shows how RCA fulfils ISO 9001:2015 corrective action requirements (Clause 10.2) and provides the investigative backbone of a CAPA process in regulated industries such as pharmaceuticals, medical devices and food safety.
