What Is Root Cause Analysis and Why It Matters at WorkUntitledA Practical Problem-Solving Discipline

Root Cause Analysis (RCA) is a structured problem-solving discipline that identifies the fundamental reasons problems, defects and failures occur, instead of treating the symptoms. Root Cause Analysis training in New Zealand equips quality, operations, engineering and safety professionals with a complete toolkit, from the 5 Whys and the Fishbone (Ishikawa) diagram to Fault Tree Analysis, FMEA and the 8D methodology, so they can investigate issues and prevent recurrence. This is a practical, non-certification skills programme with no exam. Delivered over one intensive day of 8 hours by experienced trainers, it moves beyond theory with hands-on exercises and real case studies. You learn to define a problem precisely, collect evidence, drill to the true root cause, and plan corrective actions that hold. RCA underpins ISO 9001 corrective action (Clause 10.2), the Analyse phase of Six Sigma DMAIC, Lean operations and ITIL problem management. Whether you work in food and dairy processing, healthcare, utilities, aviation or IT services, these skills apply on the job straight away. Build the problem-solving capability your team needs with Invensis Learning.

    For Individuals

    For Individuals

RCA training helps professionals move from reactive firefighting to structured, permanent problem-solving. It suits quality engineers, operations and production leaders, health and safety managers, Six Sigma practitioners and IT service managers who investigate problems and manage corrective actions. Whether you are tightening defect investigation in a dairy plant, improving patient-safety reviews, or preventing repeat IT incidents, you gain a toolkit that applies directly to your day-to-day work.

By the end of the day you can define a problem clearly, run a 5 Whys or Fishbone analysis, complete an 8D report, and verify that your corrective action actually worked. These are practical, workplace-ready skills you keep for good, with no exam to sit.

Market Pressures Making RCA a Must-Have Skill in NZ

    Costly Recurring Defects

Food, dairy and manufacturing sites lose time and margin to the same nonconformances returning. RCA gets teams past quick fixes to the underlying cause so defects stop repeating.

    RCA builds structured investigation skills

    Tougher Food-Safety Audits

FSSC 22000, BRC and Risk Management Programme audits expect documented root cause analysis behind every corrective action. RCA skills make that evidence rigorous and defensible.

    RCA supports audit-ready corrective action

    Rising Workplace Safety Expectations

Under the Health and Safety at Work Act, organisations must investigate incidents and near misses properly. Structured RCA finds systemic causes, not just human error to blame.

    RCA strengthens incident investigation

    Ageing Assets and Downtime

Utilities and infrastructure operators face equipment failures that halt production. Fault Tree Analysis and FMEA help engineers pinpoint failure pathways and prevent recurrence.

RCA builds failure and reliability analysis

    Firefighting Instead of Fixing

Many teams stay stuck reacting to the same issues without ever removing the cause. RCA replaces symptom-fixing with a disciplined method that delivers lasting improvement.

RCA embeds a root cause culture

    Scarce Structured Problem-Solvers

New Zealand has plenty of technical talent but fewer people trained to run 8D and Fishbone investigations. RCA capability sets professionals apart in quality and operations roles.

RCA makes your skills stand out

RCA is more than a technique. It is the skill that stops problems coming back. Join New Zealand professionals who investigate defects, incidents and failures with structure and confidence. Start your journey today.

    Your New Zealand Skills Transformation

Your Before and After with RCA Skills

The Shift from Symptoms to Root Causes

New Zealand employers increasingly separate teams that fix problems for good from teams that keep firefighting the same issues. As audits tighten and continuous improvement spreads, structured RCA skills are what put you on the right side of that line. Here is the shift this training delivers.

    Today

Limited to symptom-level fixes and repeated rework

    After RCA

Confident applying FTA, FMEA and 8D to complex, multi-cause failures

    Today

Overlooked for quality and improvement opportunities

    After RCA

Ready for quality, continuous improvement and reliability roles across NZ

    Today

Problem-solving value that is hard to prove to employers

    After RCA

In-demand skills valued in food, healthcare, utilities and IT services

    You master RCA

    Before

You fix the same defects and incidents again and again

    Now you have

You eliminate root causes so the same problems stop recurring

    Before

Investigations that rely on opinion, guesswork or blame

    Now you have

An evidence-based method using 5 Whys, Fishbone and 8D

    Before

Corrective actions that struggle to pass audits

    Now you have

Structured corrective action that satisfies ISO 9001 Clause 10.2

    Before

Being seen as a doer rather than a problem-solver

    Now you have

Recognition as a trusted problem-solver and workshop facilitator

The RCA Tools You Will MasterHands-On Skills for Real Investigations

This programme is built around the tools you will actually use on the job. By the end of the day you can select and apply the right RCA method for any problem, from a quick 5 Whys to a full 8D investigation, and prove your corrective action works.

Problem Definition and Is / Is Not Analysis

Write precise, factual problem statements and use Is / Is Not analysis to narrow down possible causes before investigation begins. This is where most RCA efforts succeed or fail, so you learn to separate what you observe from what you assume.

    The 5 Whys Technique

Apply iterative why questioning to drill from a symptom to the root cause of simple, single-cause problems. You learn how to reach a systemic cause, avoid stopping at blame, and verify the cause chain, plus when the 5 Whys is the wrong tool to use.

    Fishbone (Ishikawa) Diagrams

Facilitate structured, team-based brainstorming across the 6M categories to map all potential causes of complex, multi-cause problems. You practise running a Fishbone workshop and drilling each branch to the most likely root causes with evidence.

    Fault Tree Analysis and FMEA

Use top-down Fault Tree Analysis with AND / OR logic for safety-critical events, and apply FMEA to prioritise corrective actions by Risk Priority Number. These tools handle the complex failures that simpler techniques cannot resolve on their own.

    The 8D Problem-Solving Methodology

Work through all eight disciplines, from team formation and interim containment to root cause, permanent corrective action and prevention of recurrence. You complete an 8D report for a realistic scenario, the standard used across automotive and manufacturing.

    Corrective Action and Verification

Develop corrective actions that address the verified root cause, then confirm they actually worked. You learn the difference between verification and validation, and how RCA supports ISO 9001 Clause 10.2 and CAPA in regulated industries.

Is RCA training a good investment for my team?

Structured RCA reduces repeat defects, incidents and downtime, which are costly and time-consuming. Teams that share one method close corrective actions faster and pass audits more easily. Because exercises use your own real problems, the training often pays back through immediate improvements. Request a quote to discuss.

    What payment options are available?

We accept major credit and debit cards and bank transfers, and offer flexible options for corporate bookings. Speak with a learning advisor to arrange the payment method that best suits you or your organisation.

    Assessment

Is there an exam for the RCA course?

No. This is a non-certification skills programme with no exam. Learning is reinforced through hands-on RCA tool exercises, case study investigations and team discussion throughout the one-day programme, so you build capability by applying the tools rather than sitting a test.

How is my learning assessed during the training?

There is no formal assessment. Instead, you complete practical activities such as an Is / Is Not exercise, a 5 Whys investigation, a Fishbone workshop and a full 8D report for a realistic scenario. These exercises let the trainer support you as you apply each method.

Do I receive anything on completion?

Yes. On completing the one-day programme you receive a course completion certificate from Invensis Learning. This recognises that you have completed the training and covered the full RCA toolkit, though it is not a formal certification from an external body.

Will these skills expire or need renewing?

No. RCA skills do not expire and there is nothing to renew. The tools and methods you learn are immediately applicable and stay useful throughout your career, across any industry or role that involves investigating and preventing problems.

How does RCA support ISO 9001 and audits?

ISO 9001 Clause 10.2 requires organisations to determine the causes of nonconformities and act to prevent recurrence. RCA gives you a structured, documented way to meet that requirement, and the same rigour supports food-safety and health-and-safety audits in New Zealand.

    Course Content

What topics are covered in the RCA syllabus?

The syllabus spans six modules: RCA principles and problem types, problem definition and data collection, the 5 Whys, the Fishbone diagram and advanced tools (FTA and FMEA), the 8D methodology, and corrective action, verification and building an RCA culture.

How long is the RCA training programme?

The programme runs for one day, a total of 8 hours. It can be delivered as a single full day or split into two half-days, covering all six modules with concept explanations, worked examples and hands-on RCA exercises throughout.

Are the sessions practical or theory-based?

They are highly practical. Every tool is taught with worked examples and then applied in hands-on exercises and case study investigations. You practise defining problems, running 5 Whys and Fishbone analyses, and completing an 8D report, so you can use the methods confidently at work.

Can beginners follow the course content?

Yes. The course introduces every RCA tool from first principles, so beginners can follow it comfortably. At the same time, experienced quality and operations professionals gain depth in FTA, FMEA and 8D that goes beyond the basics they may already know.

What materials and resources are provided?

You receive structured courseware covering all six modules, worked examples for each RCA tool, case study scenarios, and practical templates such as Fishbone diagrams and 8D report forms that you keep and reuse in your own investigations.

    Enrollment

How do I enrol in the RCA course?

You can enrol through the Invensis Learning website. Choose your preferred format, complete your booking, and you will receive access to the schedule, courseware and joining details. To discuss options, talk to a learning advisor about upcoming sessions.

What delivery formats are available in New Zealand?

The course is available as online (live virtual) instructor-led training, in-person classroom training, and corporate onsite delivery for teams. This lets professionals and organisations across New Zealand choose the format that fits their schedule and location.

    How soon can I start?

Public sessions run on a regular schedule, and corporate onsite training can be arranged to match your team's availability. Contact a learning advisor to see upcoming batches and pick a date that works for you.

Can I arrange training just for my team?

Yes. Corporate group training is delivered onsite or live virtual on a schedule that suits your organisation, using examples relevant to your teams. It is the most effective option when you want everyone to share the same RCA method. Ask about corporate training for your team.

What is the reschedule or cancellation policy?

Reschedules and cancellations are handled per our published policy, and flexible options are available for both individual and corporate bookings. Reach out to our support team for the details that apply to your enrolment.
