    There is no formal assessment. Instead you apply multiple RCA tools to realistic scenarios throughout the programme, including a 5 Whys investigation, a Fishbone workshop and an 8D report, so you leave able to use each method at work.

    Yes. By the end you can define a problem precisely, choose the right tool, facilitate 5 Whys and Fishbone sessions, apply Fault Tree Analysis and FMEA, complete an 8D report and verify corrective actions aligned to ISO 9001:2015 Clause 10.2.

    Does this course lead to a formal qualification?

    No formal qualification or licence is awarded, because RCA is a skills discipline rather than a credential. What you gain is practical capability and a Course Completion Certificate, both valued by employers who need rigorous corrective action.

    What topics does the RCA course cover?

    The course covers RCA principles and problem types, problem definition and data collection (Is/Is Not, timelines), the 5 Whys, the Fishbone diagram, Fault Tree Analysis, FMEA in RCA, the full 8D method, and corrective action, verification and building an RCA culture.

    What RCA tools will I learn and when to use each?

    You learn five core tools: the 5 Whys for simple single-cause problems, the Fishbone diagram for complex multi-cause problems, Fault Tree Analysis for safety-critical events, FMEA for prioritising actions, and 8D for team-based corrective action. The course teaches how to select each one.

    How long is the RCA training?

    The programme runs for one day (eight hours) and can also be delivered as two half-days. It covers all six modules with concept explanations, worked examples and hands-on exercises throughout.

    Are there practical exercises?
