    You will learn the 5 Whys, the Fishbone (Ishikawa) diagram, Fault Tree Analysis, FMEA in the RCA context, the 8D methodology, Is / Is Not analysis and Pareto prioritisation, plus how to choose the right tool for each problem.

    The syllabus covers RCA principles and problem types, problem definition and data collection, the 5 Whys, the Fishbone diagram and advanced tools, the 8D problem-solving methodology, and corrective action, verification and building an RCA culture.

    Are there hands-on exercises?

    Yes. Hands-on exercises run throughout the day, including an Is / Is Not activity, a 5 Whys investigation, a Fishbone workshop and completing an 8D report for a realistic problem scenario.
