What Is Root Cause Analysis Training?UntitledYour Toolkit for Lasting Problem Solving

Root Cause Analysis Training in Tanzania is a practical, one-day programme that equips professionals with a complete Root Cause Analysis (RCA) toolkit to investigate problems, uncover their true underlying causes, and implement corrective actions that prevent recurrence. Instead of fixing symptoms and watching the same defect, incident or failure return, you learn to trace a problem to the fundamental reason it was able to happen. The course moves through precise problem definition and evidence gathering, the 5 Whys, the Fishbone (Ishikawa) diagram, Fault Tree Analysis, FMEA in the RCA context, and the 8D problem-solving method, then on to corrective action planning and verification. Hands-on exercises and case studies run throughout, so you apply each tool to realistic scenarios during the day. RCA is a core discipline in quality management, and the training directly supports the ISO 9001 Clause 10.2 requirement to determine causes and prevent recurrence. It suits quality engineers, production supervisors, operations and safety professionals across Tanzania's manufacturing, mining, food and beverage, and healthcare sectors. On completion you receive a course completion record from Invensis Learning and skills you can apply immediately at work.

Benefits of Building Real Problem-Solving SkillsThat You Can Apply from Day One

    For Individuals

    For Individuals

RCA training helps professionals build a practical problem-solving capability they use every week. Whether you handle defects on a factory line, investigate safety incidents, or manage quality non-conformances, you learn a repeatable method that gets to the real cause. The programme suits quality engineers and managers, production supervisors, operations leads, health and safety officers, and process improvement practitioners who want to stop problems returning.

If you are tired of temporary fixes and repeat failures, RCA gives you a structured toolkit that pays for itself in reduced rework, fewer incidents and better audit outcomes. You leave able to run an investigation end to end, from problem definition to verified corrective action.

    • Investigate problems methodically instead of guessing at causes and firefighting the same issues
    • Choose the right tool for the problem, from the 5 Whys to Fishbone, FTA, FMEA and 8D
    • Write precise problem statements using Is / Is Not analysis before jumping to solutions
    • Facilitate team investigations and lead structured 5 Whys and Fishbone sessions with confidence
    • Complete an 8D report with containment, root cause, corrective action and recurrence prevention
    • Develop corrective actions that address the real cause and verify they actually work
    • Support ISO 9001 Clause 10.2 corrective action in a documented, defensible way
    • Gain a workplace skill that transfers across manufacturing, mining, healthcare and services
    For Organizations

    For Organizations

For organisations, RCA group training turns problem-solving from an ad hoc effort into a disciplined capability. Teams learn to contain issues quickly, find true root causes, and put corrective actions in place that hold, reducing the cost of poor quality. The training can be delivered for quality, operations, maintenance or cross-functional teams, using your own defects and incidents as live case studies.

If your teams keep fixing the same problems, RCA training builds the structured method and facilitation skills they need to break the cycle. You gain consistent investigation practice that supports compliance, safety and margin across every unit.

    • Cuts the cost of recurring defects, rework, scrap and repeat incidents across the business
    • Builds a shared problem-solving language so teams investigate consistently
    • Strengthens ISO 9001 and TBS audit readiness through documented corrective action
    • Improves patient, worker and product safety by preventing recurrence, not just reacting
    • Standardises 8D and CAPA practice across sites and shifts
    • Develops in-house facilitators who can lead investigations without external help
    • Supports a continuous improvement culture linked to Kaizen and Six Sigma work
    • Can be tailored to your own real problems during the hands-on exercises

The Problems Tanzanian Operations Face That RCA Solves

    Costly Recurring Defects and Rework

Factories and FMCG plants lose margin to scrap, rework and repeat complaints when teams fix symptoms. RCA traces the defect to its true cause so the failure stops for good.

    RCA builds structured investigation skills

ISO 9001 and TBS Compliance Pressure

TBS certified 139 firms in the Lake Zone alone in 2025/26. ISO 9001 Clause 10.2 demands documented root cause analysis and corrective action to keep certification.

    RCA supports corrective action requirements

    Quality in Mineral Processing

As Tanzania shifts from raw export to refineries and smelters, mineral processing plants need structured RCA to control yield loss, defects and process failures at scale.

RCA builds yield and defect control skills

    Patient Safety in Healthcare

Hospitals and clinics must investigate incidents and near misses. RCA provides the disciplined method to identify systemic causes and prevent the same harm recurring.

RCA gives a systematic incident method

    Food and Beverage Safety Audits

Breweries, bottlers and food producers face HACCP and FSSC audits where every non-conformance needs rigorous root cause investigation and verified corrective action.

    RCA strengthens non-conformance investigation

    Structured Problem-Solving Gap

Many teams firefight the same issues month after month. Few have trained facilitators who can run 5 Whys, Fishbone and 8D investigations to break the cycle.

    RCA develops in-house facilitators

Problem-Solving Tools You Will MasterPractical Techniques You Can Use at Work

The skills covered in RCA training take you across the full problem-solving toolkit, from defining a problem to verifying the fix. By the end of the one-day programme you can select the right tool for a problem and run an investigation from start to finish.

    Precise Problem Definition

Learn to write a clear, factual problem statement and use Is / Is Not analysis to set the boundary of a problem before investigating, so your team solves the right issue and not a vague one.

    The 5 Whys Technique

Apply iterative why questioning to drill from a symptom to the underlying cause, recognise when an investigation has stopped too early, and avoid blame by pointing to systems and processes, not people.

    Fishbone (Ishikawa) Diagram

Facilitate a team brainstorming session across the 6M categories, Man, Machine, Method, Material, Measurement and Mother Nature, to surface and organise every possible cause of a complex, multi-cause problem.

    Fault Tree Analysis and FMEA

Use top-down Fault Tree Analysis with AND and OR logic for safety-critical events, and apply FMEA to prioritise corrective actions by severity, occurrence and detection for higher-risk problems.

    The 8D Problem-Solving Method

Work through all eight disciplines, from team formation and interim containment to root cause, permanent corrective action, recurrence prevention and closure, the standard used across manufacturing and supply chains.

    Corrective Action and Verification

Develop corrective actions that address the verified root cause, tell verification from validation, and meet ISO 9001 Clause 10.2 and CAPA expectations with documented, defensible investigation.

Root cause analysis training is a practical, one-day programme that teaches a complete RCA toolkit to investigate problems, find their true underlying causes and prevent recurrence. It is designed for quality engineers and managers, production supervisors, operations and safety professionals, and anyone who investigates defects, incidents or failures at work.

Yes. As Tanzanian employers push for value addition, ISO 9001 certification and lower cost of failure, professionals who can run structured investigations are in demand on quality, operations and safety teams. The skill is immediately useful and transfers across manufacturing, mining, food and beverage, and healthcare.

No. The course has no formal prerequisites and is open to all. Some familiarity with quality or process improvement is helpful but not required, as the training introduces RCA methods and tools from first principles with hands-on exercises throughout.

The programme covers precise problem definition and Is / Is Not analysis, the 5 Whys, the Fishbone (Ishikawa) diagram, Fault Tree Analysis, FMEA in the RCA context, and the 8D problem-solving method, plus corrective action planning and verification.

The course is available as live online instructor-led training, classroom sessions, and onsite delivery for teams. You can choose the format that fits your schedule, and corporate groups can arrange training at their own premises.

    General

What is root cause analysis training and who is it for?

Root cause analysis training is a practical, one-day programme that teaches a complete RCA toolkit to investigate problems, find their true underlying causes and prevent recurrence. It is designed for quality engineers and managers, production supervisors, operations and safety professionals, and anyone who investigates defects, incidents or failures at work.

Is RCA training worth it for professionals in Tanzania?

Yes. As Tanzanian employers push for value addition, ISO 9001 certification and lower cost of failure, professionals who can run structured investigations are in demand on quality, operations and safety teams. The skill is immediately useful and transfers across manufacturing, mining, food and beverage, and healthcare.

Do I need prior experience or qualifications to attend?

No. The course has no formal prerequisites and is open to all. Some familiarity with quality or process improvement is helpful but not required, as the training introduces RCA methods and tools from first principles with hands-on exercises throughout.

Which RCA tools does the course cover?

The programme covers precise problem definition and Is / Is Not analysis, the 5 Whys, the Fishbone (Ishikawa) diagram, Fault Tree Analysis, FMEA in the RCA context, and the 8D problem-solving method, plus corrective action planning and verification.

How is RCA training delivered in Tanzania?

The course is available as live online instructor-led training, classroom sessions, and onsite delivery for teams. You can choose the format that fits your schedule, and corporate groups can arrange training at their own premises.

    Pricing

How much does root cause analysis training cost?

Pricing varies by delivery mode, group size and whether you enrol as an individual or a corporate team. Because rates depend on your chosen format, exact pricing is available on request. Contact a learning advisor for a quote tailored to your needs.

What is included in the course fee?

The fee typically covers instructor-led training across all six RCA modules, structured courseware and reference materials, hands-on exercises and case studies, and a course completion record from Invensis Learning. Ask your advisor for the full inclusions on your chosen format.

Do you offer group or corporate rates?

Yes. We offer corporate group training with customised schedules and volume-based pricing for quality, operations and cross-functional teams. Request a quote and we will tailor a package for your organisation.

    What payment options are available?

We accept major cards and bank transfers, and can arrange invoicing for corporate bookings. Speak with a learning advisor to choose the option that suits you or your organisation.

Is the training good value for the cost?

Yes. A single recurring defect or incident often costs far more than the training itself in rework, scrap, downtime or audit findings. By teaching teams to fix problems once rather than repeatedly, RCA training tends to pay for itself quickly. Request a quote to weigh it against your own cost of failure.

    Assessment

Is there an exam for this course?

No. This is a non-certification skills course with no examination. Learning is reinforced through hands-on RCA tool exercises, case study investigations and team discussion during the one-day programme.

What do I receive after completing the training?

You receive a course completion record from Invensis Learning. There is no formal credential from an external body, as this is a practical skills programme, but the RCA methods and tools are immediately applicable in any industry.

How is my learning assessed during the course?

There is no formal assessment. Instead, you apply the tools throughout the day through an Is / Is Not exercise, a 5 Whys investigation, a Fishbone workshop, an 8D report and a corrective action plan, so you practise each method on realistic scenarios.

Will the training help with ISO 9001 corrective action?

Yes. RCA is the investigative backbone of corrective action. The course shows how to meet the ISO 9001 Clause 10.2 requirement to determine causes and prevent recurrence with a structured, documented approach that stands up to audit.

Will I be able to lead investigations after the course?

Yes. Because you practise every tool during the day, you leave able to define a problem, run a 5 Whys or Fishbone session, complete an 8D report and verify corrective actions. Many learners begin facilitating investigations for their teams straight away.

    Course Content

What will I learn in the RCA training syllabus?

You will learn to define problems precisely, collect evidence, apply the 5 Whys, Fishbone, Fault Tree Analysis, FMEA and the 8D method, and develop and verify corrective actions. The course also covers building a lasting problem-solving culture at work.

How long is the course and what is the format?

The programme runs for one day, around eight hours, and can also be split into two half-day sessions. It combines concept explanations, worked examples and hands-on RCA exercises so you apply the tools as you learn them.

Are there practical exercises and case studies?

Yes. Hands-on exercises and case study investigations run throughout the day, and corporate groups can use their own real defects and incidents as live case studies for immediate workplace value.

Which industries is the training relevant to?

RCA applies wherever problems must be prevented from recurring, including manufacturing and FMCG, mining and mineral processing, cement, food and beverage, healthcare, energy and utilities. The tools are the same across every sector.

How does this differ from Six Sigma or 7 QC Tools training?

RCA training is a focused, standalone deep dive into problem investigation tools, including FTA and 8D, at greater depth than the Analyse phase of Six Sigma or the Fishbone and Pareto coverage in 7 QC Tools. The courses complement each other well.

    Enrollment

How do I enrol in RCA training in Tanzania?

You can enrol through the Invensis Learning website or by contacting a learning advisor. Choose your preferred format, confirm your booking, and you will receive joining details, the schedule and your course materials.

What training schedules and modes are available?

We offer live online instructor-led sessions, classroom training and onsite delivery for corporate teams, with schedules to suit working professionals. Ask about upcoming batches or arrange a private group session.

Can you deliver the training at our workplace?

Yes. Onsite corporate delivery brings the training to your premises and can be tailored to your sector and your own problems. It is an effective way to train a whole team consistently. Request corporate training to arrange it.

How soon can my team start?

Public sessions run on a regular schedule, and corporate sessions are arranged around your availability, often within a few weeks. Talk to a learning advisor to confirm the next available dates for your preferred format.
